How it works

How Rehabilitation Works

  • Enquiry

    The process begins with a no obligation initial enquiry, where together we discuss your clients needs, helping determine if Think Therapy 1st is the right fit for the client at this time – if so a formal instruction for assessment can be made (include client name, address, phone number & email, dob, injury details and information on single or joint instruction). 

  • Assessment and goal setting

    A structured assessment of the clients injuries, challenges, goals and their needs. Booked within 2 working days of receiving the formal instruction and usually completed within two weeks from Instruction dependent on client availability. 

  • Personalised Rehabilitation Plan

    A bespoke rehabilitation program, integrating rehabilitation into daily routines. The report and any recommendations are sent within 10 working days of the assessment being completed. 

  • Referral Plan

    Referrers review the report and recommendations and to proceed with the rehab plan make a formal instruction for the work proposed – if this is done within 3 weeks of the assessment the same assessing SROT will likely be available (this cannot be guaranteed if after 4 weeks have passed).  

  • Rehabilitation in Action

    Rehabilitation is delivered where it matters most- home, school, work and in the community. The first session is booked within 48 hours and everyone notified. 

  • Progressive Complexity

    Rehabilitation is gradually increased in difficulty to build confidence and skill. 

  • Multidisciplinary Integration

    Close coordination with all services from day one ensures full-spectrum recovery support. 

  • Ongoing Review and Adaptation

    Rehabilitation is adjusted as progress is made, with the level of support reducing as independence grows or additional support is embedded. 

  • Reports

    Monthly update reports are provided and if needs change a new proposal of work will be provided for consideration. 

  • Therapeutic Ending

    When all goals are achieved and maintained, (or removed because full function is not going to return) the SROT consolidates the progress made and a future-proofing plan is created to ensure long-term success. Clients either move into Key working for ongoing support or transition out of active rehabilitation, with the maintenance plan in place.
